
Insurance and Billing
At Lifestrength Physical Therapy, we appreciate that the cost of medical services can be difficult to understand. Our goal is to make the billing process as easy and transparent as possible so you aren’t hit with a ‘surprise” bill.
Cost
As part of our service to you, Lifestrength Physical Therapy, Inc. will submit insurance claims on your behalf. Depending on your health insurance benefits different costs can be expected. For plans with a deductible, we will collect at time-of-service the estimated allowed per-visit cost to apply towards your overall balance (deductible). We try to keep abreast of these amounts and apply them to each particular visit during your Plan of Care. This prevents a “surprise bill” of a larger amount at the end of treatment.
*For Aetna or Blue Cross plans, you might be subject to additional financial responsibility for the initial evaluation or re-evaluation.
*We make our best effort to prevent over-collecting amounts along the way, but any credit at the end of treatment will be refunded.
*We also offer Self-Pay (Cash) rates.
General Breakdown
Initial Evaluation: $160 for plans with deductibles that have not been met.
Follow Up Appointments: Average $70-$93 for plans with deductibles that have not been met
Co-Insurance:
90/10 (Patient responsible for 10%): $5-15 per visit
80/20 (Patient responsible for 20%): $10-25 per visit
Self Pay:
Initial Evaluation: $160
Follow Up Appointment: $96
Patient Credit
If a credit accumulates on your account, it will be applied towards future visits until your Plan of Care is Complete.
*Please Note: It can take 6-8 weeks for claims to process.
Cancellation Policy
We understand that life can interfere with your planned schedule. As a courtesy, Lifestrength does not charge for the first missed appointment (no-show or late cancellation within 24-hours of scheduled appointment time).
After your second No-Show or Cancellation, within 24-hour of scheduled appointment time, we will charge a $45 No Show fee.
